Veterinary Referral Form:  Needed for therapeutic massage on a dog or cat.  Please have your Veterinarian fill out and sign.  Email back to us or bring to your first treatment session.  Please note that all animals that will have hands-on contact with a massage practitioner should be up to date on vaccinations, reasonably clean, and free of fleas/parasites.
